Is this only for hotels I haven’t stayed at yet?

It applies to any upcoming stay, whether you booked last week or six months ago, as long as the reservation hasn’t been completed yet.

What if the hotel is fully booked, will an upgrade still be possible?

Upgrades are based on availability at check-in, so they’re never guaranteed. But amenities like breakfast, credits, and welcome gifts typically aren’t dependent on availability the same way.
